Privacy policy statement, Pledge of Confidentiality, and Agreement on payment of participation fee

The Sapporo Summer Symposium for One Health (SaSSOH) organizing committee is committed to respect the importance of member personal information to protect them in an appropriate manner for their retention and management according to our Privacy Policy as follows. The SaSSOH organizing committee requires the following confidentiality agreements in order to prevent third parties from divulging the contents of presentations made at SaSSOH. The SaSSOH organizing committee requires the following consent regarding the payment of the participation fee.

1. Collection of personal information

The SaSSOH organizing committee may collect personal information to the extent necessary for execution of business purposes. Meanwhile, personal agreements by suppliers are required with our specified purpose of collection and usage for collection of personal information as a general rule.

2. Use and providing of personal information

The SaSSOH organizing committee uses acquired personal information within the range of our business purposes. The collected personal information may not be disclosed and provided to a third party without personal consent unless otherwise exceptional circumstances. In this regard, in case of 1) Based on condition of laws, 2) With the supplier's approval, 3) Out delegation on all or part of personal information handling to the extent necessary for execution of the business purpose, we can disclose and provide apply personal information.

3. Management of personal information

The SaSSOH organizing committee is committed to manage the personal information we collect with felicity in order to prevent unauthorized access, property loss, loss such as being lodged, destruction, manipulation and leaking or other factors. In the event those situations occur, we will take measures immediately to avoid their recurrences even as retrieving personal information. Moreover, when entrusting an authorized third contractor for operation within the limit of use purpose based on personal information, we are committed to manage and supervise them so that adequate administration may be performed. However, personal information released by suppliers themselves or already released to public is exempt from our privacy management.

4. Consent to Use of Photographic Images

Registration and participation in SaSSOH constitutes an agreement by the registrant for use and distribution (both now and in the future) of the registrant or attendee's image or voice in photographs, videotapes, electronic reproductions and audiotapes.

5. Pledge of Confidentiality

I AGREE to treat confidentially all Information disclosed in SaSSOH, and not to disclose it to any third party, either within or outside of Hokkaido University, EXCEPT FOR:
（1） the information/contents which, by written document(s), you can prove that the information/contents belongs to yourself, prior to the disclosure.
（2） the information/contents which have been already publicly known, prior to the disclosure.
（3） the information/contents which, by the reasons that can not come to your own responsibility, is/are publicly disclosed, posterior to the disclosure.
（4） the information/contents which you lawfully acquire from any third party having legal authority, without any confidentiality obligation being imposed upon yourself.
（5） the information/contents which, by written document(s), you can prove to invent/acquire by yourself, independently from the information/contents disclosed in this meeting.
I AGREE NOT TO TAKE any photos or videos of the presentations during the meeting.
I further ACKNOWLEDGE AND AGREE that unauthorized use or disclosure of confidential information disclosed in the meeting may result in a disciplinary action being taken or legal action being initiated.

6. Agreement to payment of participation fee

Registration and participation in SaSSOH constitutes agreement to pay the Participation Fee. If the Participation Fee has not been paid by the designated date, you will not be able to participate in this meeting.
